Johns Hopkins Bayview says 3,182 patients’ records were uploaded to unapproved AI tool

Johns Hopkins Bayview Medical Center is notifying 3,182 patients that information about their care may have been improperly shared with an outside artificial intelligence tool after a hospital worker uploaded a spreadsheet to a platform that had not been approved for use, the hospital said.

The spreadsheet covered people admitted or treated at the Baltimore hospital from May through September 2026. Information in it may have included patients’ names, admission and treatment dates, medical record and account numbers, the departments that treated them, medications administered and provider names.

The hospital said the file did not contain Social Security numbers, financial account information or other identifying information. It said it discovered the issue on Sept. 21 and opened an investigation. During an internal review that day, a staff member uploaded the spreadsheet to the unapproved tool.

Johns Hopkins Bayview said the worker removed the prompt, conversation thread and document from the tool’s history. The hospital also asked the AI company to delete the data and said it introduced measures to prevent similar incidents. It is reviewing its policies and retraining staff on protected health information and which apps and AI tools are authorized.

The hospital began mailing notification letters on Oct. 22. Patients can enroll in 12 months of free credit monitoring through Experian IdentityWorks, according to the hospital’s notice.

Patients with questions can call the hospital’s toll-free line at 1-833-931-7766 and provide engagement number B174716. The call center is open Monday through Friday, 8 a.m. to 8 p.m. Central time.
